1583 Form

The United States Postal Service requires any person or company handling for it’s customers, mail forwarded from the US Postal Service, to have for each cutomer a document entitling them to act as an agent on the customers behalf for that purpose. This is a mandatory requirement and therefore, your account will not be active until we receive the ORIGINAL 1583 form signed by you the customer and notarized.

Notaries can be found at US Consulates, embassies, as well as many other locations. You will have to mail us the ORIGINAL FORMS : faxed or emailed forms will not be accepted. This is because the US Postal Service must have your original signed 1583 form before delivering any mail for you to our place.
